Disclosure Notice of Privacy Practices

TRISCENDNP (“we” or “us”) is committed to keeping your personal information private. The Gramm-Leach-Bliley Act instituted special provisions whereby organizations are required to disclose how they share your nonpublic personal information. The purpose of this letter is to share our privacy practices with you.
In the course of providing various business services to you and your employer we collect certain nonpublic personal information about you. The nonpublic personal information we collect may include your social security number and various financial account balances, as well as information regarding your income and financial assets. These measures include computer safeguards and secured files and buildings.

We share your nonpublic personal information with insurance carriers and their representatives, your employer, our affiliates, and third-party data backup providers. We share nonpublic personal information when processing your application for life insurance coverage and in servicing and maintaining your life insurance policies in conjunction with your executive benefit program. Information is shared with your employer only to the extent the information relates to the maintenance of your program – sensitive health information is not shared with your employer.

We share nonpublic personal information with our affiliates. In servicing your executive benefit program, we utilize various affiliates of TRISCENDNP to perform everyday business purposes, such as servicing your program and assets associated with your program.

In general, we do not share nonpublic personal information about former customers. We do share nonpublic personal information about former customers to the extent such information is necessary to service or maintain an existing executive benefit program. In the case of an existing program, the scope of the information sharing is limited to those
interactions with insurance carriers and their representatives, employers, and affiliates necessary to service the program. We do not share mobile phone numbers or other contact information with third parties/affiliates for marketing/promotional purposes.
